Change of Details Form
The school must be notified immediately if there is a change in address or phone number at home or work. This is to ensure contact in case of an emergency.

Medical Forms
Medical Action Plan
All children with a medical concern need an Action Plan, preferably from your child’s Medical Practitioner which outlines; expected symptoms, recommended action & specific instructions regarding administration of medication.

The School’s Medical Policy is an important document as it provides us with the relevant information on how to best care for your children. We encourage you to make us fully aware of all medical conditions and/or requirements of your children and to inform us of any changes which may occur.

All children with a medical concern need an Action Plan, preferably from your child’s Medical Practitioner which outlines; expected symptoms, recommended action & specific instructions regarding administration of medication.  If your child has a medical condition other than those listed below, please contact the front office to discuss.

 

Medication Request
As a result of the increased number of children in schools being prescribed medications on a long and short-term basis, the educational, medical and legal authorities are continually updating their recommendations on practices and procedures for administration of medication in schools.

In line with the Catholic Education Office policy, all decisions regarding the storage and administration of medication will be the responsibility of the Principal.

External Service Providers in Schools

Although the NDIS funds a range of specialised supports for school-aged children with disability, the purpose of the scheme is not to duplicate or take over responsibilities of the Education System. Schools will continue to provide reasonable adjustments for students with disabilities to enable them to access the curriculum on the same basis as their peers.

Parents/guardians may seek to engage with external service providers when their child requires additional health, therapy, or well-being support. The services offered by the provider may align with or complement the reasonable adjustments the school provides to facilitate the student’s access to education. If the service does not link to the student’s learning needs, the service may be better delivered outside of school hours. Entry and access to the school are always at the discretion of the Principal. Acceptance of therapy services and the incorporation of service providers’ recommendations in a student’s Individualised Plan is at the Principal’s discretion.

Request to School for Third Party Clinical Access

This form is to be completed by parents / guardians to request access for external service providers, such as therapy services, for their child at school, during school hours. Please note the completion of this form does not constitute an agreement to allow for therapy to be undertaken in the school. Entry and access to the school are always at the discretion of the Principal.

Outside School Hours Care Program
The OSH Club runs a Before and After School Care program in the school.

This service provides children with fun experiences that they will enjoy. From exciting activities and healthy and nutritious snacks to caring, motivated staff, the program meets all children’s needs.

Student Insurance
The following information relates to an insurance plan Infant Jesus School has in place to provide financial benefits to students in the event of accidental injury.  The coverage is for any time the student is engaged in school activities, including travel to and from such activities.
